LIBRARY CLEARANCE
Before a clearance can be issued, any requirements or outstanding obligations to the library, such as overdue fines, unreturned materials, and payment/replacement of lost materials, must be settled.
Application for University Student Clearance shall be processed through the online university clearance system:

Avoid sending the clearance forms to all signatories simultaneously.
COLLEGE CLEARANCE (LOA, Transferring, etc.)
- Obtain a clearance form from the CEM Office of the College Secretary (OCS) by sending an email to socem@up.edu.ph. Ensure that the form is filled out accurately and all instructions are followed accordingly.
- Send it first to your adviser, then the department. Once the verification process is complete, the department will either send the clearance form to us or return it to the client.
- As per your department’s guidelines, you may also send the form to the CEM library’s email address (cemlibrary.uplb@up.edu.ph) for signature. If all requirements are met, your clearance will be signed and returned to you. Alternatively, if you have any outstanding obligations, you will be notified accordingly.
GRADUATE CLEARANCE
(Undergraduates and Graduate Students)
Congratulations on your upcoming graduation! We kindly remind you to ensure that you have met all obligations to the library before submitting your clearance form for signature.
- Obtain a clearance form from the CEM Office of the College Secretary (OCS) by sending an email to socem@up.edu.ph. Ensure that the form is filled out accurately and all instructions are followed accordingly.
- Send it to your adviser first for signature then to your department. Once the verification process is complete, the department will send the clearance form to us.
- If all requirements are met, your clearance will be signed and sent back to you.
CEM Library Requirement: Complete the CEM Library Graduate Exit Survey & Manuscript Submission Form. No need to provide us with a hardbound copy of your manuscript.

APPLICATION FOR ID REPLACEMENT
- Secure clearance form from the Office of the University Registrar (OUR).
- Complete the clearance form, attach Form-5 for the current semester/summer, and send it to the CEM library’s email address (cemlibrary.uplb@up.edu.ph) for verification and signature.
- If all requirements are met, your clearance will be signed and will be forwarded to the General References Section of the University Library for verification and signature. You will be included in the email communication to keep you informed of the progress.
